In order to help us enter you into our automated system, we ask you to pre-register before your first appointment. Download the Dental Health History form and the Patient Registration form. Complete them and FAX them to our office at (940)-380-1329. If you are unable to FAX the information, please fill them out and bring them with you. Plan to arrive 5-10 minutes early for your first visit, as there are two additional forms that will need to be signed. We will also verify the information on these forms at that time. |
